In order to expedite your visit, please bring the following items:
New Patients, in order to finalize your registration process, please arrive 30 minutes before your scheduled appointment time.
Insurance: If your insurance is a HMO or Managed Medicare, please request a referral from your Primacy Care Provider.
Please note, if you received a recall letter, please call the office at (940) 387-7588, so that we may also schedule your lab/procedure/test for you.
